This Department is part of Stephenson County in Freeport, Illinois 61032.
The function of this department is to provide long-term care to residents of Stephenson County and surrounding areas. We offer 24 hours of skilled and intermediate nursing care as well as Rehabilitation Therapies. We are the only certified Alzheimer’s facility in the area. In addition, we are Veteran Administration (VA) contracted. We employ over 160 employees and welcome volunteers.
Freedom of information must be made in writing. Requests may be made via snail mail, e-mail, special courier, or other means as long as it is in writing. However, medical records are subject to HIPAA and may not be released without proper authorization.
